REGISTRATION GUIDELINES
All participants are required to register via the Online Registration Form. Please read the following instructions and guidelines carefully to complete your registration.
PTCOG-NA Membership & Renewal:
All PTCOG-NA members with a valid paid membership will receive complimentary registration to the PTCOG-NA Symposium. For those who purchased their last membership when registering for the annual conference in Miami be sure to renew your membership prior to registration to receive complimentary registration. New members are also invited to complete your membership prior to event registration.
RENEW OR PURCHASE YOUR PTCOG-NA MEMBERSHIP HERE
Each participant will receive a Registration Number by email after completing a registration via the Online Registration Form. The preferred method of payment is by online card payment. As for payments completed by bank transfer, please proceed with the payment transfer only after receiving your registration number. Payment by bank transfer must include the registration number and participant’s name in the payment details.

* The discounted registration fees offered to members and trainees are only available to those providing valid credentials requested via the registration form. The standard registration fee will apply to those unable to provide valid details requested.
Unpaid or partially paid registrations will not be considered as valid until paid in full.

Fees are inclusive of all applicable taxes and surcharges for attendees from the United States, Canada or international countries outside of Europe.
If Attending from Europe
Please note that if you are by chance attending from Europe additional taxes may apply dependent on your location and situation in one of the following ways:
• For all attendees from an EU country, who are not VAT payers, the fee is subject to 21% CZ VAT
• For all attendees from an EU country with a valid EU VAT number, the reverse charge system will apply. The fee is subject to 0% CZ VAT and the VAT payer is to pay VAT by his or herself.
• For all attendees from the Czech Republic, regardless if you are a VAT payer or not, the fee is with 21% CZ VAT.
• For all attendees from the Czech Republic, regardless if you are a VAT payer or not, the fee is with 21% CZ VAT.
Registration Confirmation
Each participant will receive a registration overview and email notification confirming receipt of payment. To view the status of your payment, click here Online Registration Form.
Group Registrations
Group registrations can be processed online under one email address identification; you will be able to add each participant under the same invoice at the end of the registration. When registering ten or more persons under one company identity/invoice, please send an e-mail to registration@ptcog-na.org to confirm the preferred invoicing settings and inclusions.

All fees are to be paid in USD ($), free of all bank charges.

The preferred method of payment is payment by card online via our secure payment gateway:

Accepted Credit / Debit Cards
In the unlikely event of a rejected on-line credit/debit card payment, the delegate is recommended to return to his/her registration online and try to process the payment again. Should the payment issue persist, the participant is advised to contact the card issuer/bank first to check for any potential block on the online transaction. If so, once this is removed it will be possible to pay via the online payment gateway.
By Bank Transfer
To ensure your registration is valid, please make sure that any payment made by transfer is free of all bank fees and transfer charges. Please also ensure that the registered participant’s name and Registration Number are clearly stated in the bank transfer. Payments received without such details (Name, Registration Number) cannot be easily identified and may not be valid.
Payment by bank transfer is available until 24 November 2020. After this date, all payments can only be done by credit/debit card.

Beneficiary
Registration fees are collected on behalf of PTCOG-NA by the association secretariat CZECH-IN s.r.o., 5. května 65, CZ-14021 Prague 4, Czech Republic.
Final Invoice
In the event that you require a payment invoice, and the registration and payment confirmations don’t cover this, a final invoice will be provided to the participants only upon request. The Secretariat will not send this document automatically. Please contact us to ask for your final invoice if required at registration.symposium@ptcog-na.org. The final invoice will be issued based on details provided during registration (step Contact/Invoice details). Any changes of these details (address, VAT number etc.) are only possible within 3 days from the date of registration and is subject to the 15 USD administration fee.
Cancellation / Refund Policy
Cancellation of Registration
• Up to the end of November 24, 2020 a $20 cancellation fee will be deducted from all refunds
• From November 25, 2020 – no refunds are available
General Cancellation Conditions
• All cancellations must be notified in writing (by email or fax) to the PTCOG 2020 Online Secretariat and cannot be done online or over the phone.
• Requests for refunds made after November 25, 2020 will not be considered.
• It is possible to change the name/contact of the registered participant for an administration fee of $15.
Insurance
The registration fee does not include participant’s insurance against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to take out adequate personal insurance if desired.
Cancellation or Modification of the Online Meeting due to Force Majeure
In the event of a force majeure situation, CZECH-IN S.R.O. reserves the right to alter or cancel the symposium without prior notice, including any change of the time and/or venue of the symposium. Any occurrence of force majeure event shall be communicated as soon as reasonably possible from the decision time. Force majeure includes any circumstance beyond the reasonable control of CZECH-IN S.R.O., which prevents or impedes the realization of the conference. This includes but is not limited to, government action, war or hostilities, riot or civil commotion, plague or other epidemic such as SARS, bird flu, earthquake, flood, hurricane, cyclone, fire or other natural physical disaster, explosion, accident or breakdown, strike, lack of the usual means of transportation or terrorism. CZECH-IN S.R.O. shall not be liable for any direct or indirect, incidental or consequential, punitive or exemplary damages, losses, expenditures or any other inconveniences or costs caused by such modification or cancellation of the conference. Unless provided otherwise by the applicable mandatory laws, the registration fees will be reimbursed in accordance with the PTCOG-NA symposium cancellation policy.
